ALIVE + WELL FOUNDATION

The Alive+Well Foundation (AWF) is a 501c3 Foundation established in 2017 by Dr. Galen Cole and provides scholarships to support individuals who can not afford to pay. The steps in the AWF mental health services support process are: 1) An individual or group reaches out and informs us of an individual who has a mental health issue that can be addressed by outpatient psychotherapy and mental fitness training; 2) Our clinical director conducts a brief evaluation to determine the individual a) needs, b) ability to pay, and c) willingness to participate, 3) If the individual meets our criteria and is willing to participate we identify the appropriate “preferred providers” who are a) in private practice, b) fully licensed, c) practice evidence-based care, and d) have completed our 2-day training in “Trauma-Informed Evidence-Based” care, 4) Once we have determined that an individual is a good fit for our program and secured funding to pay for treatment, we match this person with our “preferred providers” and pay for ten in-patient sessions and five mental fitness sessions. NOTE: The cost of managing and treating an individual with a mental health issue like major depression, general anxiety, panic disorder, and PTSD is $5,375. This covers ten in-patient sessions, five mental fitness sessions, and the cost of managing the process. When you donate, you will receive a tax-deductible receipt for 75% ($4,031.25) of your donation, sent directly to the Foundation. You will also receive a receipt for the remaining 25% ($1,343.75). However, this amount will not be tax deductible as it goes directly to the organization managing this treatment process, CCS Psychotherapy, Inc.
